Instagram and Facebook ads for med spas without getting your account restricted

Meta's cosmetic procedure and before-after rules, the creative that gets approved, offers worth running, budgets, and a retargeting build for aesthetics.

Instagram is the primary discovery channel for aesthetics and Meta's ad policies are the least friendly in local business to what aesthetic clinics want to show. Those two facts define the whole channel. You cannot skip it, and you cannot run it the way your competitors' feeds suggest they do.

The first thing to accept: Meta restricts ads that focus on an individual's body or appearance in a way that implies a negative self-perception, and it restricts cosmetic procedure content, particularly side-by-side before and after imagery. A rejected ad is not a bug. A restricted ad account after enough rejections is not a bug either, and getting one back takes weeks you do not have.

Creative that gets approved and still sells

The line to work inside: show the result, do not sell the insecurity. Meta's reviewers are reacting to the framing as much as the image.

What works:

  • Single-frame result photos. One after image, natural light, full face or full frame, no split screen, no arrows, no circles drawn on a problem area. Caption it as the treatment, not the flaw.
  • Injector on camera. A 20 to 40 second clip of your provider explaining what a treatment does and who it is not for. This is the highest-performing format in aesthetics right now, and it is the one Meta almost never rejects.
  • Client testimonial video. The client speaking about the experience, the room, the provider. No before shot cut in.
  • Room and process footage. Cleaning the treatment area, opening a sealed syringe, the consultation itself. It answers the safety question first-time clients actually have.
  • Offer cards. Plain text and brand color, no body imagery at all. These are the safest creative you can run and they carry retargeting well.

What gets you rejected: side-by-side collages, zoomed-in close-ups of a treated area, text like "fix your hooded eyes" or "hate your double chin," anything implying weight loss on a specific body, and skin close-ups with severe acne on the left panel.

Offers that fill a treatment calendar

Discount-first offers train a client base to wait for the next discount. In a business where the whole model is repeat visits, that is expensive.

Better structures, roughly in order of how well they hold up:

  1. Free consult with a treatment credit. "Book a complimentary consult this month and receive $50 toward your first treatment." It gets the person in the room, where your close rate is what it is, and the credit costs nothing if they do not book.
  2. Membership trial. First month of the membership at a reduced rate. Attracts the client you actually want, the one who returns monthly. The structure is in the pricing and memberships guide.
  3. New client bundle. A specific package at a specific price, framed as a value rather than a percentage off. "New client facial and dermaplane, $149."
  4. Event. A Thursday evening with the injector, limited to 20 spots, priced normally. Events work in aesthetics because scarcity is real and social proof is visible.

Avoid per-unit pricing in ad creative. It attracts unit shoppers, it collides with brand-name policy issues, and it anchors your whole market down.

Budget and structure

A workable starting build for a single-location clinic:

  • Cold prospecting, $25 to $50 a day, one campaign, broad targeting inside a 10 to 15 mile radius, age 28 to 60, no interest stacking. Meta's delivery finds aesthetic buyers better than an interest list built from device brand names.
  • Retargeting, $10 to $15 a day, split into website visitors in 30 days, Instagram engagers in 90 days, and video viewers past 50%.
  • Client list, $5 to $10 a day, uploaded customer list for rebooking pushes and membership offers. This is usually the cheapest revenue in the account.

Expect $12 to $45 per lead cold, and $4 to $15 per lead on retargeting. Those leads are softer than Google Ads leads because the person was not searching. Budget the follow-up labor accordingly, and read the speed to lead guide before you turn anything on. A Meta lead that sits for four hours is worth a fraction of one answered in five minutes.

Retargeting is where the money is in aesthetics

Aesthetic purchases have a consideration window measured in weeks. Someone watches a filler explainer, thinks about it, looks at your grid, checks your reviews, and books three weeks later. Cold ads get credit for none of that unless retargeting carries them across.

Build three audiences and let them run continuously:

  • Video viewers past 50% in the last 90 days. Show them the offer.
  • Website visitors in the last 30 days who did not book. Show them the provider on camera and the reviews.
  • Everyone who booked a consult but did not treat. Show them a specific, simple next step.

The third audience is the most valuable list in the account and the one most clinics never build.

The organic side still matters

Prospects who find you through Google check the Instagram grid before calling. An account whose last post is from March is a reason not to book. You do not need daily posts. You need a standard: three posts a week, one of which is the injector's face, and stories on the days you are open.

Reviews and Instagram do different jobs and reinforce each other. Across 53,312 med spa listings, 45.1% show a perfect 5.0, so the grid is often what actually separates two clinics that both look flawless in the map pack. The rest of that picture is in the benchmark report.

My before and after posts do fine organically. Why not in ads? Organic content is reviewed under Community Standards. Paid content is reviewed under the advertising policies, which are stricter, and cosmetic procedure imagery is specifically called out there.

Should I boost posts from the app? No. Boosted posts cannot use proper objectives, audiences or placements, and they burn budget on engagement rather than bookings. Run everything from Ads Manager.

Do I need a different account for ads if mine got restricted? Fix the underlying creative first. Spinning up new business accounts to escape a restriction is the fastest way to lose the page as well.
